Best Practice for Housing Sector in Health and Social Care Integration

The availability of appropriate good quality housing and housing services has been identified as making an important contribution to the success of the integration of health and social care.
There are already a number of excellent examples of local authorities, housing associations and third sector organisations such as Housing Options Scotland and Care and Repair Scotland, working to deliver a better quality of life for their customers by linking housing services with health and social care initiatives.
Key areas such as prevention, housing options advice, personalisation, technology, homelessness and adaptations are all relevant when considering the role of housing in health and social care integration.
Some examples of good practice are detailed below:
- Inverclyde Council working with Registered Social Landlords and Occupational Therapists to develop much clearer transparent processes across Community Health and Care Partnerships around housing allocation.
- Dundee City Council working with Caledonia Housing and Bield Housing to increase in the number of alternative care/housing options.
- South Ayrshire Council project looking at targeted housing adaptations to support hospital discharge.
- Borders Council looking at alternative housing models and intermediate care.
- Rowan Alba and Bield work together to offer a personalised service at Thorntree
