She was the Minister when I started at the beginning of 2023, so she knows the team and we know her, and she is passionate about women’s health. I met with her in early June, where we discussed many of the topics in the Plan but especially abortion, eliminating cervical cancer, menopause and heart disease. Although a number of organisations have expressed concern that we no longer have a dedicated Minister with “Women’s Health” in their title, the subject sits squarely in Ms Todd’s portfolio and I am confident that she is totally committed to improving the health of women and girls in Scotland.

Phase Two of the Plan was launched at the end of January with four ‘new’ priority areas – transforming gynaecology services, eliminating cervical cancer, brain health and innovation. But the work of Phase One continues and still underpins much of what the team and I do. We continue to work on menstrual health and menopause.

I have been involved in planning, with the Royal Society of Edinburgh, a Round Table on menstrual health and wellbeing in the autumn. This roundtable is designed to inform the implementation of the Scottish Women’s Health Plan: Phase Two (2026–2029) and support the Scottish Government’s developing approach to menstrual health. The session will explore how planned menstrual health commitments can be delivered in a holistic, evidence‑informed, and experience‑led way. It will also consider how responsibilities are shared across health services, education, workplaces, and wider sectors.

We had a face-to-face meeting with the Women’s Health Leads from many of the Boards in early June. We heard about ongoing initiatives from each of the Boards and had a detailed presentation from Alice Ritchie (Grampian) on their Community Appointment Day and from Lisa Adams on the work she has been doing on cervical screening in prisons (of which more later). We also discussed how to improve the quarterly Leads meetings, and I think we have a plan for focussing the meetings more directly on the actions of the Plan. It was great to see everyone in person and there was lots of sharing of ideas.

I was lucky enough to attend the International Brain Health Conference in Edinburgh in May. I learned a lot. Significant advances are being made in the diagnosis of dementia/Alzheimer’s disease (AD) and there are some great examples of specialist brain health clinics around the UK to offer investigation and support to people who are affected.

There is much work going on to search for effective treatments for dementia /AD. While a ‘cure’ still seems a long way off, in anticipation of improved therapeutic options, Scotland needs to focus on the development of high quality brain health services in anticipation of improvements in diagnosis and treatment. Meanwhile, much of the focus (and the focus for the WHP) is on preserving brain health and I heard an interesting presentation on nutrition and brain health and was persuaded that a mediterranean diet is beneficial for the brain if not for the bank balance. The effect of the menopause, of contact sports and of intimate partner violence were also discussed in detail.

While the literature suggests that estrogens have a clear role in modulating dementia risk, the evidence for a mitigating effect of HRT (delaying the onset or reducing the risk) is mixed with some studies reporting a beneficial effect, others a deleterious effect and some reporting no effect. Yet ‘influencers’ and some clinicians are telling women that HRT definitely prevents dementia. The new Health of Women Research Network will be inaugurated with a study group meeting on October 19th on women, dementia and HRT with presentations from experts followed by brainstorming on research ideas and opportunities.

The Strategic Action Plan for Eliminating Cervical Cancer was published in March. The bottom line is that the uptake of both HPV vaccination and screening needs to increase significantly throughout Scotland with efforts focussing on deprived areas.

I spoke about the Women’s Health Plan at a meeting at New College Lanarkshire recently. In passing we spoke about cervical cancer, and the college has committed to raising awareness among their students, staff and the local community of the importance of cervical screening and immunisation as preventative measures in reducing the risk of cervical cancer. We met with the college and colleagues from the eliminating cervical cancer team and from Public Health Lanarkshire to brainstorm ideas for action. The enthusiasm of the New College staff is impressive – and infectious – and we will work closely with them to drive forward a number of projects, including, I hope, an event offering simultaneous meningococcus vaccination and HPV catch up vaccination for eligible students at the start of the new term. I hope that the work with New College may serve as a pilot for similar actions to be rolled out to all further education colleges throughout Scotland.

Over a year ago the team identified the female prison population as a group less likely to have had HPV vaccination and to have been screened for cervical cancer.

Dr Lisa Adams, our public health specialty registrar, has led a project on cervical screening in Scottish prison settings. Lisa and the Women’s Health Plan team have held a number of workshops and undertaken surveys among both prisoners and staff aimed at understanding their views about women’s health in general, screening and cervical cancer and prison‑specific arrangements for screening. I have now visited Stirling and Polmont prisons a couple of times and held meetings with prisoners to explain the importance of cervical cancer, our ability to prevent and treat it, the inequities among women from deprived areas of Scotland and what actually happens when a woman is screened. These sessions have been great fun but also rewarding. Used to speaking to groups of medical students apparently reluctant to ask questions, I found the women in prison interested, engaged and very funny. They were also extremely grateful that we were taking an interest in them and, importantly, after each session women who have never been screened or whose screening is long overdue presented to the prison health service for screening. While in the grand scheme of things screening more women in prisons is not going to amount to huge numbers, we feel that they are a very deserving population and that their views probably reflect those of other underserved groups.

I hope you are all enjoying the summer.

Anna Glasier

The Women’s Health Team and I are looking forward to progressing the actions within Phase Two of the Women’s Health Plan.
