{"id":448,"date":"2014-10-10T10:11:55","date_gmt":"2014-10-10T10:11:55","guid":{"rendered":"http:\/\/blogs.scotland.gov.uk\/health-and-social-care-integration\/?p=448"},"modified":"2016-08-26T11:29:26","modified_gmt":"2016-08-26T10:29:26","slug":"best-practice-for-housing-sector-in-health-and-social-care-integration","status":"publish","type":"post","link":"https:\/\/blogs.gov.scot\/health-social-care\/2014\/10\/10\/best-practice-for-housing-sector-in-health-and-social-care-integration\/","title":{"rendered":"Best Practice for Housing Sector in Health and Social Care Integration"},"content":{"rendered":"<p>The availability of appropriate good quality housing and housing services has been identified as making an important contribution to the success of the integration of health and social care.<\/p>\n<p>There are already a number of excellent examples of local authorities, housing associations and third sector organisations such as <a href=\"http:\/\/www.housingoptionsscotland.org.uk\/\">Housing Options Scotland<\/a> and <a href=\"http:\/\/www.careandrepairscotland.co.uk\/\">Care and Repair Scotland<\/a>, working to deliver a better quality of life for their customers by linking housing services with health and social care initiatives.<\/p>\n<p>Key areas such as prevention, housing options advice, personalisation, technology, homelessness and adaptations are all relevant when considering the role of housing in health and social care integration.<\/p>\n<p>Some examples of good practice are detailed below:<\/p>\n<ul>\n<li><a href=\"http:\/\/www.jitscotland.org.uk\/example-of-practice\/closer-working-housing\">Inverclyde Council working with Registered Social Landlords and Occupational Therapists to develop much clearer transparent processes across Community Health and Care Partnerships around housing allocation.<\/a><\/li>\n<li><a href=\"http:\/\/www.jitscotland.org.uk\/example-of-practice\/housing-care\">Dundee City Council working with Caledonia Housing and Bield Housing to increase in the number of alternative care\/housing options.\u00a0\u00a0<\/a><\/li>\n<li><a href=\"http:\/\/www.jitscotland.org.uk\/example-of-practice\/targeted-housing-adaptations\">South Ayrshire Council project looking at targeted housing adaptations to support hospital discharge.<\/a><\/li>\n<li><a href=\"http:\/\/www.jitscotland.org.uk\/example-of-practice\/alternative-housing-models-intermediate-care\">Borders Council looking at alternative housing models and intermediate care.<\/a><\/li>\n<li><a href=\"http:\/\/www.ccpscotland.org\/hseu\/news\/rowan-alba-bield-work-together-offer-personalised-service-thorntree\/\">Rowan Alba and Bield work together to offer a personalised service at Thorntree<\/a><\/li>\n<\/ul>\n<p>For more information, email <a href=\"mailto:margaret.irving@scotland.gsi.gov.uk\">Margaret Irving<\/a> in the Scottish Government Housing Directorate or contact on 0141 242 5441.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>The availability of appropriate good quality housing and housing services has been identified as making an important contribution to the success of the integration of health and social care.
