{"id":554,"date":"2014-11-28T15:50:52","date_gmt":"2014-11-28T15:50:52","guid":{"rendered":"http:\/\/blogs.scotland.gov.uk\/health-and-social-care-integration\/?p=554"},"modified":"2016-08-26T11:59:17","modified_gmt":"2016-08-26T10:59:17","slug":"optimising-the-quality-of-life-for-older-people","status":"publish","type":"post","link":"https:\/\/blogs.gov.scot\/health-social-care\/2014\/11\/28\/optimising-the-quality-of-life-for-older-people\/","title":{"rendered":"Optimising the Quality of Life for Older People"},"content":{"rendered":"<p align=\"left\"><a href=\"http:\/\/www.healthscotland.com\/\" target=\"_blank\">NHS Health Scotland<\/a> has developed an <a href=\"http:\/\/www.jitscotland.org.uk\/resource\/older-peoples-outcomes-framework\">Outcomes Framework<\/a> for Older People working with the Scottish Government\u2019s <a href=\"http:\/\/www.scotland.gov.uk\/Topics\/Health\/Support-Social-Care\/Support\/Older-People\/ReshapingCare\">Reshaping Care for Older People<\/a> (RCOP) policy team, the Scottish Government Analytical Services Division, the <a href=\"http:\/\/www.jitscotland.org.uk\/\">Joint Improvement Team<\/a> and local partnerships.<\/p>\n<p align=\"left\">The work is closely linked to the<a href=\"http:\/\/www.evaluationsupportscotland.org.uk\/how-can-we-help\/shared-learning-programmes\/stitch-time\/\" target=\"_blank\"> \u2018Stitch in Time\u2019<\/a> project, led by <a href=\"http:\/\/www.evaluationsupportscotland.org.uk\/\" target=\"_blank\">Evaluation Support Scotland <\/a>on the third sector contribution to Reshaping Care for Older People.<\/p>\n<p align=\"left\">The aim of the Outcomes Framework is to:<\/p>\n<ul>\n<li>develop a \u2018big picture map\u2019 of the main pathways to achieving the high level outcomes identified for older people<\/li>\n<li>bring together and\u00a0summarise the existing relevant evidence and indicators for older people<\/li>\n<li>strengthen existing\u00a0mechanisms for planning and evaluating services, whether through joint\u00a0commissioning or community planning<\/li>\n<li>understand the contribution of partnership activities to delivering outcomes.<\/li>\n<\/ul>\n<p align=\"left\">View the written reports of the framework\u2019s two main components the <a href=\"http:\/\/www.jitscotland.org.uk\/wp-content\/uploads\/2014\/10\/Outcomes-Framework-Book-STRATEGIC-MODEL.pdf\" target=\"_blank\">strategic outcomes model<\/a> and four\u00a0<a href=\"http:\/\/www.jitscotland.org.uk\/wp-content\/uploads\/2014\/10\/Outcomes-Framework-Book-NESTED-MODEL.pdf\" target=\"_blank\">nested models<\/a> (which illustrate a range of preventative services) and the diagrams of the nested models on <a href=\"http:\/\/www.jitscotland.org.uk\/wp-content\/uploads\/2014\/10\/Outcomes-Framework-Visual-NESTED-MODEL-Age-Friendly-Homes.pdf\" target=\"_blank\">age-friendly homes<\/a>, <a href=\"http:\/\/www.jitscotland.org.uk\/wp-content\/uploads\/2014\/10\/Outcomes-Framework-Visual-NESTED-MODEL-Eat-Well.pdf\" target=\"_blank\">eating well<\/a>, <a href=\"http:\/\/www.jitscotland.org.uk\/wp-content\/uploads\/2014\/10\/Outcomes-Framework-Visual-NESTED-MODEL-Falls-Prevention.pdf\" target=\"_blank\">falls prevention<\/a>, <a href=\"http:\/\/www.jitscotland.org.uk\/wp-content\/uploads\/2014\/10\/Outcomes-Framework-Visual-NESTED-MODEL-Quality-of-End-of-Life.pdf\" target=\"_blank\">end-of-life<\/a>, and <a href=\"http:\/\/www.jitscotland.org.uk\/wp-content\/uploads\/2014\/10\/Outcomes-Framework-Visual-STRATEGIC-MODEL-Main.pdf\" target=\"_blank\">strategic outcomes <\/a>model for optimising quality of life.<\/p>\n<p align=\"left\">For more information, contact <a href=\"mailto:eilidh.smith@scotland.gsi.gov.uk\">Eilidh Smith<\/a>.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>NHS Health Scotland has developed an Outcomes Framework for Older People working with the Scottish Government\u2019s Reshaping Care for Older People (RCOP) policy team, the Scottish Government Analytical Services Division, the Joint Improvement Team and local partnerships.
