Supplementary Form For Cremation Applications – Update

We previously blogged about when to use the supplementary form for applications for cremation on 3 April and on 17 September.

 From 5 January 2021 mainland Scotland has been moved from level 4 to a temporary lockdown which is likely to mean that it will again be more difficult for funeral directors and cremation authorities to obtain the signatures of applicants.

In order to avoid any possible delays to cremations and funeral services the downloadable supplementary form can be used by funeral directors when they are unable to obtain a signature from the applicant.

Funeral directors are reminded that they should never sign the cremation application form itself.  They should use the supplementary form instead, stating in the relevant section why it has been used.  Cremation authorities should anticipate an increase in use of the supplementary form in the coming weeks.
